Job summary

Affinity Hospice is seeking a results-driven Hospice Consultant to drive referral growth within an assigned territory. You will develop trusted relationships with physicians, hospitals, long-term care facilities, and community partners, serving as a key ambassador for Affinity Hospice in a field-based role.

You will educate referral sources on hospice eligibility and benefits, identify opportunities, and collaborate with clinical teams.

Qualifications

• 2+ years of healthcare sales or marketing experience.
• Preference for hospice/home health experience.
• Proven ability to build relationships, influence decision-makers, and drive results.
• Strong territory planning, time management, and organizational skills.

Responsibilities

• Build and maintain referral relationships with physicians, hospitals, and care facilities.
• Educate referral sources on hospice eligibility, benefits, and Affinity’s differentiators.
• Identify, qualify, and close referral opportunities by engaging key decision-makers and overcoming objections.
• Own and manage territory using data, market insights, and performance metrics.
• Document referral activity and support community education and outreach.
