Qualifications

• 3+ years of leadership experience, with marketing background preferred in hospice.
• 3+ years of marketing experience.
• Highly data-driven with ability to manage marketing programs across channels.
• Excellent verbal and written communication at all levels.
• Ability to educate healthcare professionals, patients, and families about home care services.
• Proven ability to build relationships with referral sources.

Responsibilities

• Establishes and maintains contacts with social workers, discharge personnel, doctors' offices, nursing homes, and other useful referral personnel.
• Proactively seeks opportunities to meet with referral sources to build referrals.
• Identifies educational opportunities with contracted skilled nursing facilities and other senior care facilities to raise awareness of the agency's programs.
• Develops and maintains relationships with key contractors, discharge planners and other facilities to provide attentive customer service.
• Coordinates patient admissions with the team.
• Conducts on-site visits to assess patient needs and identify barriers to care.
• Keeps accurate records of patient interactions and referrals (CRM).
• Achieves admissions, product, and payor-mix goals.
• Responds to customer complaints in a timely manner.
• Performs other duties as assigned.
