Job summary

Choice Hospice, Inc. seeks a Hospice Field Registered Nurse (RN) PRN in Phoenix, AZ.

This role delivers patient-centered, home-based nursing care, coordinating care plans with the interdisciplinary team to ensure comfort and dignity for patients and support for their families. You will perform initial and ongoing assessments, educate patients and families, support care planning, and provide on-call coverage.

Qualifications

• Current RN license
• CPR certification
• Valid driver’s license
• Valid auto insurance
• At least 2 years Hospice RN experience preferred

Responsibilities

• Perform initial and ongoing comprehensive assessments of patient needs.
• Educate patients and families regarding disease process, care plans, and end-of-life care.
• Coordinate and implement the plan of care with the IDG to ensure quality, continuity, and timely revisions.
• Provide clinical directions to Hospice Aide and LPN/LVN; ensure documentation and communication.
• Respond to patient and family needs with on-call coverage as assigned.
