Job summary

Choice Health At Home in Glendale, AZ is seeking a Hospice Field Registered Nurse (RN) to deliver compassionate, patient-centered care in homes and care settings. You will assess needs, develop and manage care plans, collaborate with the IDG, and educate patients and families about end-of-life care and comfort.

The role requires current RN license, CPR certification, valid driver’s license, and experience in hospice nursing.

Current RN License

Valid Auto Insurance
At least 2 years of Hospice RN experience preferred
HomeCare HomeBase experience preferred

Responsibilities

Initial and ongoing comprehensive assessments of the patient's needs and development of the care plan.
Implement and revise the plan of care to ensure quality and continuity of care.
Educate patient and family about disease process, end-of-life care, nutrition and self-care.
Coordinate scope and frequency of services based on acuity and needs.
Provide clinical directions to aides and LPN/LVN to ensure service continuity.
Prepare progress notes showing progress toward goals and maintain documentation.
Inform physicians and staff of changes in patient needs and outcomes.
Perform on-call responsibilities and provide on-call service as assigned.
