Job summary

Choice Health at Home is seeking a Hospice Field RN (PRN) in Overland Park, KS, to deliver compassionate, patient-centered hospice care in patients' homes or other settings. You will assess needs, create care plans, and coordinate with an interdisciplinary team to ensure comfort and dignity for patients and support for families.

Responsibilities include on-going assessments, care planning, staff education, and effective communication with physicians and caregivers.

Qualifications

• Current RN license required.
• CPR certification required.
• Valid driver’s license and auto insurance.
• At least 2 years hospice RN experience preferred.
• HomeCare HomeBase experience preferred.

Responsibilities

• Perform initial and ongoing assessments of patients in home or care settings.
• Develop, implement, and document plans of care with the interdisciplinary team.
• Educate patients and families about disease, self-care, and end-of-life care.
• Coordinate care, communicate changes to physicians and team members, and ensure continuity of care.
• Provide on-call coverage as assigned and document progress notes.
