Job summary

Choice Health at Home seeks a Hospice Field RN (PRN) in the Phoenix area to deliver compassionate, patient-centered care in patients’ homes. You will coordinate care with physicians, social workers, and chaplains to support comfort, dignity, and family needs.

Responsibilities include assessments, care planning, patient and family education, and providing clinical directions to aides and LPNs/LVNs. Strong clinical judgment and independent decision-making are essential in this dynamic home-care

Qualifications

• Current RN License.
• CPR Certification.
• Valid Driver’s License.
• Valid Auto Insurance.
• At least 2 years of Hospice RN experience preferred.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patients’ physical, functional, psychosocial, and environmental needs.
• Apply admission and re-certification criteria for hospice care to determine level of care.
• Develop and document the plan of care to ensure quality and continuity.
• Educate patients and families on disease, self-care, end-of-life care, nutrition and care needs; train staff as needed.
