Job summary

Choice Hospice, Inc. is seeking a dedicated Hospice Field Registered Nurse (RN) to join our Tucson, AZ team.

The role involves delivering compassionate, patient-centered nursing care in patients’ homes or other care settings, coordinating interdisciplinary plans, and ensuring comfort, dignity, and support for families. The RN will perform comprehensive assessments, develop care plans, educate patients and families, and provide guidance to the care team.

Qualifications

• Current RN license required.
• CPR certification required.
• Valid driver’s license required.
• Valid auto insurance required.
• At least 2 years hospice RN experience preferred.
• HomeCare HomeBase experience preferred.

Responsibilities

• Perform initial and ongoing assessments of patients in home or care settings.
• Develop and update plans of care to ensure quality and continuity of care.
• Educate patients and families about disease process, end-of-life care, and self-care.
• Provide clinical directions to Hospice Aide and LPN/LVN.
• Prepare progress notes and document patient outcomes.
• Participate in on-call rotation as assigned.
