Job summary

Sage Hospice Primary & Palliative Care is seeking a passionate On-Call Registered Nurse (RN) to join the team in Mesa, AZ. The role focuses on delivering skilled nursing care to patients at home, coordinating with the interdisciplinary team, and supporting families with compassionate care.

The ideal candidate will hold an RN license, have CPR certification, and bring 2+ years of clinical RN experience, preferably in hospice or home health.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and valid driver’s license.
• Two years’ RN experience in clinical care; hospice/home health preferred.
• Current CPR certification.
• Proficient computer skills; Microsoft Office experience preferred.
• Strong interpersonal skills and ability to support patient and family.
• Demonstrate skill in nursing practice and cope with family emotional stress.

Responsibilities

• Administer skilled nursing care for clients in their place of residence.
• Coordinate care with the interdisciplinary team, patient/family and referring agency.
• Assume responsibility for coordination of care and on-call duties.
