Job summary

Choice Health at Home is seeking a Hospice Field Registered Nurse (RN) PRN in Phoenix, AZ. The RN will deliver compassionate, patient-centered care in homes and other settings, coordinating plans of care with an interdisciplinary team to ensure comfort and dignity for patients and support for families.

Responsibilities include comprehensive assessments, admission criteria application, care plan development, patient and family education, and on-call coverage.

Qualifications

• Current RN license is required.
• CPR certification is required.
• A valid drivers license and auto insurance are required.
• Hospice RN experience of 2+ years is preferred.
• HomeCare HomeBase experience is preferred.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patients in home or care settings.
• Apply admission and re-certification criteria for hospice care.
• Develop and document the plan of care; revise as needed.
• Educate patients and families on disease, self-care, end-of-life care; train staff as needed.
• Assess services needed based on acuity and family needs; support caregivers.
• Provide clinical directions to Hospice Aide and LPN/LVN to ensure continuity of care.
• Prepare progress notes showing progress toward goals and ensure proper documentation.
• Inform physician and others of changes in patient needs and responses to care.
• Perform on-call duties and provide on-call care to patients and families.
