Job summary

Choice Hospice, Inc. seeks a Hospice Field RN (PRN) to deliver compassionate, patient-centered care in patients’ homes and community settings across the Phoenix area. The role focuses on comprehensive nursing care plans that promote comfort, dignity, and family support.

You will assess, plan, and coordinate care with the IDG, educate families, and provide on-call coverage while maintaining high clinical standards at the end of life.

Qualifications

• Current RN license required.
• CPR certification required.
• Valid driver's license and auto insurance.
• At least 2 years hospice RN experience preferred.
• HomeCare HomeBase experience preferred.

Responsibilities

• Perform initial and ongoing assessments of patients’ needs and plan of care.
• Develop, document, and update the plan of care for quality and continuity.
• Educate patients and families on disease, end-of-life care, nutrition, and self-care.
• Provide on-call service and coordinate with the interdisciplinary group.
• Supervise Hospice Aide and LPN/LVN as needed.
