Job summary

Choice Hospice, Inc. in Tyler, TX is seeking a Hospice Field RN to deliver patient-centered care in homes and community settings. You will assess needs, coordinate care plans, and support families during life-limiting illness.

The role requires an active RN license, CPR, a valid driver’s license and auto insurance, at least 2 years of hospice experience, and familiarity with HomeCare HomeBase to ensure high-quality care and smooth team collaboration.

Qualifications

• Current RN License
• CPR Certification
• Valid Driver’s License
• Valid Auto Insurance
• At least 2 years of Hospice RN experience preferred
• HomeCare HomeBase experience preferred

Responsibilities

• Initial and ongoing comprehensive assessments of the patient’s physical, functional, psychosocial, and environmental needs with documentation and team input.
• Apply admission and recertification criteria to establish appropriate levels of care.
• Develop and document the plan of care to ensure quality and continuity of service.
• Educate patients and families about disease process, end-of-life care, and self-care; train staff as needed.
• Determine scope and frequency of services based on acuity and caregiver capacity.
• Provide clinical directions to Hospice Aide and LPN/LVN for continuity of care.
• Prepare progress notes showing progress toward goals with proper documentation.
• Inform physician and team of changes in patient needs and response to care.
• Perform on-call responsibilities as assigned.
