Job summary

Choice Health At Home is seeking a Hospice Field Registered Nurse (RN) to deliver compassionate, patient-centered care in home settings around Garland, Texas. You’ll assess patients, coordinate comprehensive care plans, and collaborate with physicians, social workers, and chaplains to support families and uphold comfort and dignity.

This role requires strong clinical expertise, excellent communication, and the ability to make independent decisions in dynamic environments.

Qualifications

• Current RN license
• CPR certification
• Valid driver’s license
• Valid auto insurance
• At least 2 years of Hospice RN experience preferred
• HomeCare HomeBase experience preferred

Responsibilities

• Perform initial and ongoing assessments of patients' physical, functional, psychosocial, and environmental needs.
• Apply admission and recertification criteria for hospice care to establish appropriate levels of care.
• Develop and document the plan of care to ensure quality and continuity of care; revise as necessary.
• Educate patients and families about disease, home care, nutrition, end-of-life care; train staff as needed.
• Determine scope and frequency of services based on acuity and family needs; assess caregiver capability.
• Provide clinical directions to Hospice Aide and LPN/LVN to ensure continuity of service.
• Prepare progress notes and communicate changes to physicians and care team.
• Participate in on-call coverage as assigned.
