Job summary

Choice Health At Home is seeking a Hospice Field RN to deliver compassionate, patient-centered care in patients' homes and care settings in Austin, TX. You will assess needs, coordinate plans, and educate families to ensure comfort and dignity at end-of-life.

The role requires an active RN license, CPR certification, and the ability to work independently in dynamic environments; Hospice experience preferred.

Responsibilities

• Perform initial and ongoing assessments of patients' physical, functional, and psychosocial needs.
• Determine eligibility and admission criteria for hospice care.
• Develop and update the plan of care to ensure quality and continuity.
• Educate patients and families about disease, care options, and end-of-life planning.
• Provide clinical directions to Hospice Aide and LPN/LVN; coordinate care.
• Maintain thorough documentation and communicate changes to the care team; handle on-call duties.
