Job summary

Choice Health at Home is seeking a compassionate Part-Time Hospice Field Registered Nurse to provide patient-centered care in patients' homes within the Conroe, TX area. The RN will coordinate care with physicians, social workers, and other IDG members to ensure comfort and quality of life for patients with life-limiting illnesses.

Qualifications

• Current, unrestricted Registered Nurse license.
• Current CPR certification.
• Valid driver’s license.
• Reliable transportation for travel to patient care locations.
• Strong assessment, documentation, communication, and care-coordination skills.
• Ability to work independently and collaborate effectively with an interdisciplinary team.

Responsibilities

• Conduct initial and ongoing comprehensive assessments of patients’ physical, functional, psychosocial, environmental, cultural, and spiritual needs.
• Assess activities of daily living, caregiver capabilities, grief risks, and indicators of patient and family needs.
• Apply admission and recertification criteria to determine hospice eligibility and level of care.
• Develop, implement, and update individualized plans of care to promote quality and continuity of services.
• Determine scope and frequency of nursing and support services based on patient acuity and needs.
• Educate patients and families about disease process, symptom management, and end-of-life care.
