Job summary

Choice Hospice, Inc. is seeking a part-time Hospice Field RN to deliver compassionate, patient-centered care in home and community settings in Tomball, TX.

You will manage comprehensive nursing care plans, coordinate with physicians, social workers, and chaplains to support patients and families through life-limiting illnesses. The role requires current RN license, CPR certification, a valid driver’s license and auto insurance; at least 2 years hospice RN experience preferred.

Qualifications

• Current RN license in Texas.
• CPR certification required.
• Valid driver’s license and auto insurance.
• At least 2 years hospice RN experience preferred.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patient needs.
• Develop and implement the plan of care to ensure quality and continuity of care.
• Educate patients and families on disease process, end-of-life care, and self-care.
• Coordinate with IDG and inform physicians of changes in patient condition.
• Provide on-call coverage as assigned.
• Document progress and ensure timely clinical notes.
