Job summary

Choice Health At Home seeks a Hospice Field RN for evening/night on-call coverage in Tyler, TX. The role centers on compassionate, patient-centered care at home, coordinating care plans and collaborating with physicians, social workers, and chaplains to ensure comfort and dignity for patients and support for families.

You will perform assessments, plan of care, educate patients, and provide on-call service. RN license, CPR, valid driver's license, and HomeCare HomeBase experience preferred.

Qualifications

• Current RN License in good standing.
• CPR Certification required.
• Valid Driver’s License and Auto Insurance.
• Hospice RN experience preferred (2+ years).
• HomeCare HomeBase experience preferred.

Responsibilities

• Assess patients' physical, functional, psychosocial, and environmental needs.
• Determine level of care and eligibility for hospice services.
• Develop, implement, and document the plan of care and revisions.
• Educate patient and family on disease, self-care, and end-of-life care.
• Coordinate with IDG, provide staff training, and ensure continuity of care.
• Supervise Hospice Aide and LPN/LVN; provide on-call coverage.
• Document progress and communicate changes to the team.
