Job summary

Choice Health at Home is seeking a dedicated Hospice Field RN (Registered Nurse) for full-time work in Tyler, TX. The role focuses on delivering compassionate, patient-centered nursing in home and care settings, coordinating with an interdisciplinary team to ensure comfort and dignity for patients with life-limiting illnesses.

You will assess patient needs, educate families, and develop care plans while supervising aides and clinical staff to maintain high-quality end-of-life care.

Qualifications

• Current RN license is required.
• CPR certification required.
• Valid driver’s license and auto insurance required.
• Hospice RN experience and/or HomeCare HomeBase experience preferred.

Responsibilities

• Perform initial and ongoing assessments of patient needs and adjust care plans.
• Coordinate care with physicians, social workers, and chaplains.
• Provide education to patients and families about disease, end-of-life care, and self-care.
• Supervise and direct Hospice Aide and LPN/LVN to ensure quality care.
• Document progress notes and communicate changes in patient needs to the team.
