Job summary

Choice Health At Home's Hospice division in Tucson is seeking a Hospice Field RN to deliver compassionate, patient-centered care in patients' homes and other care settings. The RN will perform comprehensive assessments, develop and implement care plans, and educate patients and families.

Collaboration with the interdisciplinary group and on-call coverage are essential to ensure quality and dignified end-of-life care.

Qualifications

• Current RN license is required.
• CPR certification is required.
• Valid driver’s license is required.
• Valid auto insurance is required.
• At least 2 years of Hospice RN experience preferred.
• HomeCare HomeBase experience preferred.

Responsibilities

• Perform initial and ongoing assessments of the impact of terminal disease on patient needs and document findings.
• Apply admission and recertification criteria to establish levels of care and eligibility for hospice services.
• Develop, document, and revise the plan of care to ensure quality and continuity of care.
• Educate patient and family about disease, end-of-life care, nutrition, and self-care; coordinate with the IDG.
• Determine scope and frequency of services based on acuity and caregiver capability; provide guidance to staff.
• Provide clinical directions to Hospice Aide and LPN/LVN; ensure continuity of service.
• Prepare progress notes showing progress toward goals and maintain documentation of care.
• Inform physician of changes in patient needs and outcomes; monitor response to interventions.
• Fulfill on-call responsibilities.
