Job summary

Hospice Austin is seeking a registered nurse to provide comprehensive hospice care in homecare and facility settings. Responsibilities include admissions visits, ongoing patient assessments, developing plans of care, symptom management, and collaboration with physicians and the IDT.

The RN will supervise CNAs, LVNs, and homemakers, ensure timely documentation, support families, and participate in discharge planning and staff education.

Qualifications

• RN license in Texas required.
• CPR certification.
• Graduation from an approved school of professional nursing.

Responsibilities

• Provides professional nursing care for hospice patients in homecare and facility settings.
• Performs initial and ongoing assessments of patients and environmental needs.
• Develops, maintains, and updates individualized plans of care and manages symptoms.
• Coordinates with physicians, IDT, hospital, and facility staff for care transitions.
• Supervises CNAs, LVNs, and homemakers; ensures clinical boundaries and safety.
• Documents assessments, orders, and progress notes accurately and timely.
• Conducts admissions visits for new patients and initiates the plan of care.
• Provides ongoing education to patients, families, and facility staff on hospice philosophy and care coordination.
