Job summary

The role involves entering data, running workflows in EMR, coordinating with Clinical Managers, and training new coordinators. You will support patients and colleagues by phone, email, and fax while protecting PHI and compliance.

Qualifications

• Requires high school diploma; college degree preferred.
• Thanks minimum two years health care experience, hospice preferred.
• Demonstrates autonomy, organization, assertiveness, flexibility and cooperation.

Responsibilities

• Enter data accurately into multiple information systems.
• Run workflow functions in electronic medical record system.
• Verify data accuracy with Clinical Managers and/or Clinical Director.
• Manage incoming, outgoing and interoffice mail.
• Provide patient and colleague support by phone, email and fax.
• Alert management to potential compliance issues.
• Maintain confidentiality of PHI and organization information.
• Order DME and coordinate exchanges with suppliers.
• Participate in IDG meetings and take notes.
• Assist with office procedure projects as assigned.
• Train and support new Clinical Care Coordinators within the region.
