Job summary

Affinity Hospice Management LLC in Roanoke, VA is seeking a Clinical Care Coordinator to support the Clinical Care team by managing the electronic medical records system and handling back office tasks such as billing, payroll, HR and intake and scheduling.

The role also includes training new Coordinators, data entry, EMR workflow, DME coordination, and ensuring PHI confidentiality while providing support to patients and colleagues by phone, email and fax.

Qualifications

• High School Diploma (College Degree Preferred)
• At least two years health care experience, preferably in hospice
• Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities
• HCHB/PointCare knowledge is preferred
• Proficient in MS Office and Google Applications

Responsibilities

• Accurately enters data into information systems
• Executes workflow functions in electronic medical record system
• Verifies accuracy of data with Clinical Manager(s) and/or Clinical Director
• Manages incoming, outgoing and interoffice mail
• Provides customer support to patients and colleagues via phone, email and fax
• Alerts appropriate management regarding potential compliance issues
• Maintains confidentiality of PHI and organization information
• Orders DME and coordinates exchanges as needed
• Participates and scribes IDG meetings
• Performs other office procedures
• Provides training and ongoing support to new Coordinators within the region
