Affinity Health Management is seeking a Clinical Care Coordinator in Roanoke, VA to support the Clinical Care team and manage the electronic medical records system. The role includes administrative duties such as billing, payroll, HR, intake and scheduling, along with training new coordinators in the region.

Key tasks include data entry, EMR workflow execution, data accuracy verification, mail management, and patient/colleague support via phone, email and fax.

Formación

• High School Diploma (College Degree Preferred).
• At least two years health care experience, preferably in hospice.
• Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
• HCHB/PointClickCare knowledge is preferred.
• Proficient in MS Office and Google Applications.

Responsabilidades

• Accurately enters a variety of data into various information systems.
• Executes workflow functions in electronic medical record system.
• Verifies accuracy of data entered through collaboration with Clinical Manager(s) and/or Clinical Director.
• Manages incoming, outgoing and interoffice mail.
• Provides customer support to patients and colleagues via phone, email and fax.
• Alerts appropriate management team members regarding potential compliance issues.
• Maintains confidentiality of PHI and organization information at all times.
• Orders DME and alerts DME supplier when equipment is no longer in use/needs to be exchanged for other, etc.
• Participates and scribes IDG meetings.
• Performs other specific projects relating to office procedures.
• Provides training and ongoing support to new Clinical Care Coordinators within the region.
