Job summary

Affinity Hospice Management LLC is seeking a Clinical Care Coordinator to support the Clinical Care team by managing the electronic medical records system and handling back-office administrative tasks such as billing, payroll, HR, and intake.

The role includes providing training and ongoing support to new coordinators, ensuring data accuracy, and maintaining PHI confidentiality across regional operations.

Qualifications

• High School Diploma; college degree preferred.
• At least two years health care experience, preferably in hospice.
• Demonstrates autonomy, organization, assertiveness, flexibility and cooperation.
• HCHB/PointCare knowledge preferred.
• Proficient in MS Office and Google Applications.

Responsibilities

• Enter data accurately into various information systems.
• Execute workflow functions in the electronic medical record system.
• Verify data accuracy with Clinical Manager(s) and/or Clinical Director.
• Manage incoming, outgoing, and interoffice mail.
• Provide customer support to patients and colleagues via phone, email and fax.
• Alert management to potential compliance issues.
• Maintain confidentiality of PHI and organization information.
• Order DME and coordinate exchanges with suppliers when needed.
• Participate in and document IDG meetings.
• Assist with office-wide projects as required.
• Provide training and ongoing support to new Clinical Care Coordinators within the region.
