Job summary

Affinity Hospice Management LLC is seeking a Clinical Care Coordinator to support the Clinical Care team by managing the electronic medical records system and assisting with back-office functions such as billing, payroll, HR and intake.

The role includes training new Coordinators and providing ongoing regional support. Strong data entry, organizational skills, and proficiency in MS Office and Google Applications are essential, along with healthcare experience.

Qualifications

• High School Diploma required; college degree preferred.
• At least 2 years health care experience, hospice preferred.
• Demonstrates autonomy and organizational skills.
• HCHB/Pointcare knowledge preferred.
• Proficient in MS Office and Google Applications.

Responsibilities

• Enter data into information systems accurately.
• Execute workflow in electronic medical record system.
• Verify data accuracy with Clinical Manager(s) or Clinical Director.
• Manage incoming/outgoing/interoffice mail.
• Provide customer support to patients and colleagues via phone, email and fax.
• Alert management to potential compliance issues.
• Maintain PHI and organization confidentiality.
• Order DME and coordinate exchanges.
• Participate in IDG meetings; document outcomes.
• Support and train new Clinical Care Coordinators in the region.
• Perform other office-related tasks as needed.
