Job summary

Affinity Health Management seeks a Clinical Care Coordinator to provide administrative support for the Clinical Care team, including EMR management, billing, payroll, HR and intake. You will train new coordinators and ensure PHI confidentiality across the region.

The role requires data entry accuracy, strong MS Office and Google Apps skills, and the ability to coordinate with clinical managers. Hospice experience is preferred, with autonomy and teamwork valued.

Qualifications

• High School Diploma (College Degree Preferred)
• At least two (2) years health care experience, preferably in hospice
• Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities
• HCHB/PointCare knowledge is preferred
• Proficient in MS Office and Google Applications

Responsibilities

• Accurately enters a variety of data into various information systems
• Executes workflow functions in electronic medical record system
• Verifies accuracy of data entered through collaboration with Clinical Manager(s) and/or Clinical Director
• Manages incoming, outgoing and interoffice mail
• Provides customer support to patients and colleagues via phone, email and fax
• Alerts appropriate management team members regarding potential compliance issues
• Maintains confidentiality of PHI and organization information at all times
• Order DME and alerts DME supplier when equipment is no longer in use/needs to be exchanged for other, etc.
• Participates and scribes IDG meetings
• Performs other specific projects relating to office procedures
• Provides training and ongoing support to new Clinical Care Coordinators within the region
