Remarkable Hospice is seeking a Hospice Education Specialist to expand reach by building relationships with referral sources and educating the community about hospice care.

You will partner with clinical teams, meet monthly referral goals, and represent the organization with professionalism and compassion, delivering presentations to healthcare professionals and community stakeholders.

Qualifikationen

• High school diploma or equivalent is required; a Bachelor's degree is preferred.
• 1–3 years’ experience in sales, marketing, or business development, preferably in home care or hospice.
• Driver’s license and auto liability insurance; no listing in OIG Excluded Provider Listing; registry/licensure as required by state.

Aufgaben

• Build and maintain relationships with physicians, hospitals, SNFs, assisted living facilities, and other referral partners.
• Visit referral sources regularly to promote hospice services and awareness.
• Represent Remarkable Hospice at health fairs, community events, and networking opportunities.
• Deliver presentations to healthcare professionals and community leaders about hospice care benefits.
• Explain eligibility criteria and hospice philosophy to patients and families.
• Collaborate with intake and clinical teams to ensure smooth transitions from referral to admission.
• Meet or exceed monthly referral and admissions goals.
• Document all contacts, meetings, and activities in the CRM in compliance with HIPAA and policy guidelines.
• Perform other related duties and participate in continuing education.
