Job summary

ExcelaCare Hospice seeks a Hospice Education Specialist to expand reach by building relationships with referral sources, educating the community, and representing ExcelaCare with professionalism and compassion.

You will collaborate with intake and clinical teams to ensure smooth transitions from referral to admission and strive to meet monthly referral and admissions goals. Travel and community presence are essential to this role.

Qualifications

• High school diploma or equivalent; Bachelor’s degree preferred.
• 1–3 years in sales, marketing, or business development; home care or hospice preferred.
• Driver’s license and current auto liability insurance; no listing in the OIG Excluded Provider list.
• In good standing on state nurse aide registry or meets federal licensure requirements.
• Willingness to travel locally and maintain reliable transportation.

Responsibilities

• Build and maintain relationships with physicians, hospitals, SNFs, and other referral partners.
• Visit referral sources regularly to promote hospice services and awareness.
• Represent ExcelaCare at health fairs and community events.
• Deliver presentations to healthcare professionals and community leaders.
• Educate on eligibility criteria and ExcelaCare’s approach to care.
• Collaborate with intake and clinical teams for smooth admissions.
• Meet monthly referral and admissions goals.
• Document all contacts in the CRM per HIPAA and policy guidelines.
• Maintain confidentiality and comply with agency policies.
