Remarkable Hospice is seeking a Hospice Education Specialist to expand our reach by building relationships with referral sources, educating the community, and representing the organization with professionalism and compassion.

You will collaborate with intake and clinical teams to ensure smooth transitions from referral to admission, deliver presentations to healthcare professionals, and promote timely hospice care across our service area.

Qualifications

• Education and advocacy experience in home health, hospice, or health services.
• Ability to read, write, and present information clearly to diverse groups.
• Strong interpersonal skills and the ability to develop trusted relationships.

Responsibilities

• Build and maintain relationships with physicians, hospitals, and referral partners.
• Represent Remarkable Hospice at health fairs and community events.
• Deliver presentations to healthcare professionals and organizations about hospice care benefits.
• Collaborate with intake and clinical teams to ensure smooth transitions from referral to admission.
• Meet or exceed referral and admissions goals and document activities in CRM.
