Job summary

Enhabit Home Health & Hospice is seeking a proactive professional to manage relationships with referral sources and drive growth in post-acute care referrals. You will educate physicians, hospitals and health systems about Enhabit services and program requirements to support patient transitions and outcomes.

Ideal candidates have a healthcare business development background, strong communication skills, and a valid driver's license; clinical licensure may be required for some roles.

Qualifications

• Bachelor's degree or graduate of nursing, therapy or social work; equivalent experience considered.
• Minimum two years in healthcare business development, community relations, marketing, or sales.
• Knowledge of Medicare/Medicaid rules and home health regulations.
• Valid driver license and automobile liability insurance.
• Strong interpersonal, communication and presentation skills.
• Clinical licensure required if clinical role.
• Experience with electronic referral systems and Homecare Homebase (HCHB) preferred.

Responsibilities

• Act as primary relationship manager between Enhabit and referral sources (physicians, hospitals, health systems, senior living, etc.).
• Promote services and educate referral sources and patients to support post-acute care transitions and improve patient outcomes.
• Support referral growth and ensure high levels of referral source satisfaction.
