Job summary

Enhabit Home Health & Hospice seeks a dedicated professional to manage relationships with physicians, hospitals and other referral sources to promote post-acute care transitions. You will educate partners on Enhabit services, ensuring regulatory compliance and high-quality patient outcomes.

Ideal candidates have healthcare business development experience, strong communication skills, and a drive to build partnerships. In-office work is essential to support teamwork and service excellence.

Qualifications

• Bachelor's degree or higher in a healthcare-related field; nursing, therapy or social work graduates encouraged to apply.
• Minimum two years in healthcare business development, community relations, marketing, or sales.
• Knowledge of federal, state, and local laws governing Medicare certified home health and hospice operations.
• Valid state driver license and automobile liability insurance as required by law.

Responsibilities

• Act as the primary relationship manager between Enhabit and referral sources including physicians, practices, hospitals, health systems, and senior facilities.
• Educate referral sources and patients about Enhabit services and program requirements to support post-acute care transitions.
• Drive referral growth by increasing awareness and understanding of services to improve patient outcomes and satisfaction.
