Job summary

Enhabit Home Health & Hospice is seeking a motivated Relationship Manager to cultivate referrals from physicians, hospitals, and health systems. You will educate partners on Enhabit services, navigate post-acute care transitions, and support growth in referral channels to improve patient outcomes.

Ideal candidates have a Bachelor's in healthcare admin or related field, 2+ years in healthcare business development or sales, and a valid driver license.

Qualifications

• Bachelor's or higher in healthcare admin, marketing, or related field; or graduate of nursing/therapy/social work.
• Two+ years in healthcare business development, community relations, marketing, or sales.
• Knowledge of federal, state, and local laws governing Medicare certified home health and hospice operations.
• Valid state driver's license and automobile liability insurance.
• Strong interpersonal, communication and presentation skills.
• Clinical license if applicable for clinical roles.
• Home health, hospice, or post-acute care experience preferred.

Responsibilities

• Serve as the primary relationship manager between Enhabit and referral sources including physicians, hospitals, and health systems.
• Support referral growth by educating partners on Enhabit services and program requirements.
• Navigate post-acute care transitions to improve patient outcomes and satisfaction.
