Job summary

Enhabit Home Health & Hospice seeks a relationship-focused professional to manage referrals from physicians, hospitals, and healthcare partners. You will educate providers on services and program requirements to grow referrals and improve patient outcomes during post-acute transitions.

The role requires a healthcare business development background, strong communication skills, and a valid driver license for regional travel. In-office attendance is essential.

Qualifications

• Bachelor's degree in healthcare administration, marketing, or related field or graduate of nursing, therapy, or social work.
• Minimum two years in healthcare business development, community relations, marketing, or sales.
• Knowledge of federal/state/local laws governing Medicare home health and hospice operations.
• Must hold a valid state driver license and automobile liability insurance.
• Strong interpersonal, communication, and presentation skills.
• Clinical license if applicable per state requirements.

Responsibilities

• Serve as primary relationship manager between the company and referral sources (physicians, hospitals, health systems, and other providers).
• Support referral growth by educating on services and program requirements to referral sources, patients, families, and caregivers.
• Navigate post-acute care transitions to improve patient outcomes and satisfaction.
