Job summary

The Care Team Hospice is seeking a driven Community Care Liaison to grow our presence in Indiana by building strong referral partnerships with physicians, hospitals, and healthcare partners.

You will educate providers, patients, and families on the value of hospice, advocate for patients, and collaborate with a collaborative team to expand census while delivering compassionate, patient-centered care. Travel within the assigned geography is required to meet opportunities and trainings.

Qualifications

• Bachelor’s degree or equivalent.
• 1-2 years healthcare specific outside sales experience.
• 1-2 years of Hospice or Home Health sales preferred.

Responsibilities

• Build and expand relationships with physicians, healthcare partners, and key decision-makers to drive referral volume and market growth.
• Serve as a dedicated advocate for hospice by educating the community and sharing the value of comfort-focused, patient-centered care.
• Achieve census and admission targets by positioning our hospice as a preferred provider within the community.
• Engage regularly with healthcare providers and referral sources to create strong, collaborative partnerships focused on patient well-being.
• Lead hospice discussions with patients and families, clearly communicating services and guiding the consent process with professionalism and empathy.
