Abide Hospice Care in Portland, OR is seeking a Community Liaison to deepen relationships with healthcare professionals and referral partners throughout the Portland metro area. You will promote a presence-driven hospice message focused on compassion, dignity, and exceptional clinical care.

In this role, you’ll build trust, educate partners, and grow census through consistent outreach and meaningful partnerships, while collaborating with clinicians to ensure a seamless, patient-centered

Qualifications

• 1–3 years of hospice, home health, or healthcare sales and relationship-building experience.
• Proven ability to develop referral relationships and meet growth goals.
• Natural curiosity about people and ability to listen before speaking.
• Confidence engaging physicians, social workers, discharge planners, and families.
• Self-directed, organized approach to developing and managing referral relationships.
• Belief that people are more than a diagnosis, a referral, or a number.

Responsibilities

• Build genuine, lasting relationships with healthcare professionals and referral partners throughout the Portland metropolitan area.
• Introduce a different hospice message rooted in presence, dignity, relationships, and exceptional clinical care.
• Grow our hospice census through consistent outreach, education, and trusted referral partnerships.
• Open doors through curiosity and connection, not simply persuasion.
• Work closely with our clinical team to ensure a seamless, compassionate experience for patients, families, and referral partners.
• Represent Abide's belief that ordinary care becomes sacred when we bring extraordinary attention to the person before us.
