Job summary

Enhabit Home Health & Hospice is seeking a capable professional to serve as the primary relationship manager with referral sources. The role focuses on educating providers about Enhabit services to support smooth post-acute transitions and improve patient outcomes.

The ideal candidate has healthcare business development or sales experience, strong communication skills, and a valid driver’s license. In-office attendance is essential to meet objectives and collaborate with teams.

Qualifications

• Bachelor’s degree in healthcare administration, marketing or related field or graduate of nursing/therapy/social work program.
• Equivalent years of work experience may substitute for degree requirements.
• Must have strong interpersonal, communication and presentation skills.
• Valid driver’s license and automobile liability insurance as required by law.
• Experience with Medicare/home health regulatory guidelines is preferred.

Responsibilities

• Acts as primary relationship manager between Enhabit and referral sources (physicians, hospitals, health systems, senior communities, etc.).
• Educates referral sources about Enhabit services and program requirements to support post-acute care transitions and patient outcomes.
• Supports referral growth and maintains productive partnerships with stakeholders to ensure high satisfaction.
