Job summary

Enhabit Home Health & Hospice is seeking a proactive business development professional to manage relationships with referral sources including physicians, hospitals and health systems. The role focuses on educating partners about services, driving referral growth and supporting post-acute care transitions to improve patient outcomes.

Candidates should have a healthcare-related degree or equivalent experience, with 2+ years in healthcare business development and knowledge of Medicare regulations.

Qualifications

• Bachelor's degree or graduate of nursing/therapy/social work; equivalent experience accepted.
• Graduate of nursing/therapy/social work is highly preferred.
• Minimum two years in healthcare business development, community relations, marketing, or sales.
• Knowledge of federal/state/local laws for Medicare home health and hospice.
• Valid state driver license and automobile liability insurance.
• Strong interpersonal, communication and presentation skills.
• If clinical, must hold a clinical license per state requirements.
• Experience with home health, hospice, or post-acute care preferred.
• Experience with electronic referral systems and HCHB preferred.
• Established relationships with physicians, hospitals or senior communities preferred.
• Dependable transportation and safe driving in all weather.
• Fluency in English; good listening and communication skills.
• Microsoft Office proficiency; Word and Excel.

Responsibilities

• Serve as the primary relationship manager with referrers such as physicians and hospitals.
• Educate referral sources about Enhabit services and program requirements.
• Support referral growth and smooth post-acute care transitions.
• Help navigate patient care journeys to improve outcomes and satisfaction.
