Job summary

Cadre Hospice is seeking a Community Relations Representative to connect the care team with physicians, hospitals, SNFs, home health agencies, and other referral partners. You will educate healthcare professionals about hospice eligibility and the benefits of early involvement, building trusted relationships and expanding access to compassionate end-of-life care.

Qualifications

• Minimum 2 years in healthcare marketing, referral development, or healthcare sales; hospice experience preferred.
• Strong consultative sales and relationship-building skills.
• Knowledge of hospice or palliative care services.
• Valid driver's license and reliable transportation.

Responsibilities

• Build meaningful relationships with physicians, hospitals, SNFs, assisted living, home health agencies, and other referral partners; educate on hospice eligibility.
• Develop strategic territory plans; generate qualified patient referrals through consultative sales; expand Cadre’s presence.
• Represent Cadre Hospice at community events; educate patients, caregivers, and organizations about hospice services; advocate for earlier access.
• Collaborate with clinical and operational leaders to ensure a seamless referral and admission experience; monitor market trends.
