Three Oaks Hospice is seeking a Community Liaison to promote hospice programs and services within a designated territory. You will build relationships with physicians, hospitals, and referral sources to enroll patients in hospice in a timely manner.

The role emphasizes outreach, market assessments, and developing marketing plans to expand community understanding of hospice care. The position requires travel and collaboration across teams to ensure care excellence.

Qualifications

• Minimum 1 year of sales/marketing experience in healthcare.
• 1–2 years hospice/palliative marketing experience preferred.
• Position is field-based and requires 80% travel within assigned geographic area.

Responsibilities

• Generates patient referrals through new and existing sources.
• Maintains regular communication with referral sources to ensure service delivery.
• Identifies opportunities for additional or improved services.
• Maintains relationships to improve community understanding of hospice services.
• Develops and implements marketing plans to meet budget projections.
• Monitors community perceptions of quality and services.
• Participates in meetings and related activities as needed.
• Participates in evening/weekend call for admissions as required.
• Maintains confidentiality due to access to sensitive information.
• Very good attendance and flexibility to meet department needs.
• Follows Medicare, Medicaid and HIPAA regulations.
