Crossbridge Hospice, LLC is seeking a Community Liaison in Michigan to drive business development and manage communications with referral partners, patients, and caregivers. You will serve as the local market leader for B2B, B2C, and DTC engagement, promoting hospice services and building strong community relationships.

The role requires proven experience in healthcare marketing or outreach, strong KPI focus, and the ability to work independently in a fast-paced environment.

Qualifications

• Proven experience in healthcare marketing, sales, or community outreach—preferably in home health, hospice, or a related field.
• Demonstrated ability to meet or exceed performance goals and KPIs, including referral volume and patient admissions.
• Strong interpersonal and communication skills for diverse audiences.

Responsabilités

• Identify opportunities for in-services in the health care community and coordinate their implementation with appropriate persons.
• Build and maintain relationships with referral sources such as hospitals, physician offices, senior living communities, and other healthcare providers.
• Identify and pursue new business opportunities to grow patient census across service lines.
• Represent the organization at community events, health fairs, and professional networking functions.
• Educate patients, families, caregivers, and legal representatives on hospice benefits and services.
• Guide families through the enrollment process and ensure timely follow-up on referrals.
