Crossbridge Hospice, LLC is seeking a Community Liaison to connect our healthcare services with the community and lead business development. You will engage referral partners, patients, and families, and represent the organization at events.

The role focuses on outreach to hospitals, physician offices, and senior living communities, with a drive to grow patient census and ensure timely enrollment processes.

Qualifications

• Proven experience in healthcare marketing, sales, or community outreach.
• Ability to meet or exceed performance goals and KPIs.
• Strong relationships with referral sources and community partners.

Responsibilities

• Drive business development and manage inbound/outbound communications.
• Represent the organization to referral partners, patients, and caregivers.
• Develop relationships with hospitals, physician offices, and senior living communities.
• Identify new business opportunities to grow patient census across services.
• Educate patients and families about hospice services and enrollment.
