Job summary

Crossbridge Hospice in Grand Rapids, MI seeks a Community Liaison to drive business development and serve as the local market leader for outreach to referral partners, patients, and caregivers. This role connects hospice services with the community, expanding census across service lines while upholding our mission-driven culture.

Ideal candidates have healthcare marketing, sales, or outreach experience, strong relationship-building skills, and knowledge of payer sources; a valid driver’s license

Qualifications

• Experience in healthcare marketing, sales, or community outreach.
• Track record of meeting KPIs and growth goals.
• Knowledge of Medicare/Medicaid is a plus.

Responsibilities

• Drive business development and manage inbound/outbound communications.
• Build relationships with referral sources such as hospitals and senior living communities.
• Represent organization at community events and health fairs.
• Educate patients and families on hospice services.
• Coordinate with medical staff offices to provide hospice information.
• Monitor referrals and ensure timely follow-up.
