Job summary

Crossbridge Hospice is seeking a Community Liaison in Akron, OH to drive business development and manage communications with referral partners, patients, and caregivers. The role combines B2B/B2C engagement to expand service reach and improve patient access to care.

The candidate will build relationships with hospitals, physician offices, and senior communities, coordinate in-services, and educate families on hospice options.

Qualifications

• Experience in healthcare marketing, sales, or community outreach is required.
• Ability to build and maintain referral partner relationships.
• Strong interpersonal and communication skills across diverse audiences.

Responsibilities

• Identify opportunities for in-services in the healthcare community and coordinate with stakeholders.
• Build relationships with referral sources such as hospitals and physician offices.
• Grow patient census by pursuing new business opportunities and representing the organization at events.
• Educate patients and families on services and guide enrollment processes.
• Develop relationships with medical staff offices and hospitals; ensure timely follow-up on referrals.
