Job summary

Heart to Heart Hospice is seeking a Community Education Representative to drive hospice referrals by engaging physicians, hospitals, and senior living communities. This role focuses on developing territory plans and building relationships to support admissions.

The ideal candidate has 1–3 years of healthcare sales experience, understands Medicare Hospice Benefit, and can manage time and resources effectively while maintaining regulatory compliance.

Qualifications

• One to three years of progressive sales experience in hospice or healthcare sales.
• Knowledge of business management and Medicare Hospice Benefit.
• Strong time management, organizational skills, and ability to meet deadlines and work autonomously.
• Valid driver's license and reliable transportation with current auto liability insurance.

Responsibilities

• Call on physicians, area hospitals, SNFs, ALFs and other senior living settings to promote hospice referrals and Heart to Heart Hospice House.
• Collaborate with hospice eligible patients and their loved ones as they begin admission to hospice, in collaboration with the Hospice House staff.
• Develop and implement territory business plans and manage sales time/resources according to plan and goals.
• Establish and maintain ongoing public relations efforts to increase brand awareness and community understanding of the organization and its services.
• Utilize customer relationship management software and other tools to effectively manage accounts and performance.
• Perform other duties as required and ensure compliance with laws, Medicare regulations, and company policies.
