Job summary

Heart to Heart Hospice is seeking a Community Education Representative to drive hospice business development in a sales-focused role. You will call on physicians, hospitals, and senior living facilities to promote referrals and collaborate with Hospice House staff through patient admission processes.

The position emphasizes territory planning, public relations efforts, and effective use of CRM tools to grow the organization’s presence in the community.

Qualifications

• 1–3 years of progressive sales experience in hospice sales, home health, or healthcare sales.
• Knowledge of business management and a strong knowledge of the Medicare Hospice Benefit.
• Excellent time management and organizational skills, with the ability to meet deadlines and work autonomously.
• Valid driver’s license.
• Reliable transportation and provide ongoing valid/current auto liability insurance.

Responsibilities

• Call on physicians, area hospitals, skilled nursing facilities, assisted living facilities or other senior living settings to promote hospice referrals.
• Collaborate with Hospice House staff as patients begin admission to hospice.
• Develop and implement territory business plans and manage sales time and resources to meet goals.
• Establish and maintain ongoing public relations efforts to increase brand awareness and community understanding of the organization and its services.
• Utilize CRM software and other tools to effectively manage accounts and performance.
• Perform other duties as required.
• Ensure compliance with local, state, and federal laws, Medicare regulations, and policies.
