Job summary

Patriot Healthcare seeks a Hospice Certified Nursing Assistant to deliver personal care per the patient’s plan, under nurse supervision. You will work as part of an interdisciplinary group to implement patient care plans and ensure comfort for patients with terminal or chronic illnesses.

Qualifications

• Provide personal care per patient plan as directed by nurse supervisors.

Responsibilities

• Stewards the mission and values of Patriot Healthcare with compassionate hospice care.
• Participates with the IDG in development and implementation of the patient/family plan of care.
• Performs tasks per Hospice policies and supervisor instructions.
• Ensures patient comfort and safety; assists with activities of daily living.
• Documents tasks and communicates with family or facility staff as needed.
• Creates a warm, caring environment and maintains open communication with supervisor.
