Job summary

Patriot Healthcare is seeking a Hospice Certified Nursing Assistant (CNA) to provide personalized care according to nursing assessments and care plans under nurse supervision. You will help with daily living activities and ensure patient comfort throughout shifts.

As part of the care team, you will participate with the IDG to create and implement patient/family care plans, follow hospice policies, and document care while keeping open communication with supervisors and family members.

Qualifications

• Provide personal care services according to patient care plan.
• Contribute to development of patient/family care plans with IDG.
• Follow hospice policies and procedures and nurse instructions.

Responsibilities

• Provide patient personal care as per nursing assessments and care plan.
• Collaborate with the IDG to implement the care plan.
• Carry out tasks per hospice policies and supervisor directions.
• Maintain scheduled assignments and complete tasks as directed.
• Keep patient comfortable at all times.
• Assist with activities of daily living (mouth care, bathing, dressing, nails, etc.).
• Document care and communicate patient needs to supervisor.
