• Provide personal care services to assigned patients according to nursing assessments and care plans under nurse supervision.
• Participate with the IDG in developing and implementing patient/family care plans.
• Perform tasks per Hospice policies and supervisor instructions.
• Maintain patient comfort; ensure timely completion of tasks.
• Assist with activities of daily living and basic care functions.
• Document tasks and observations related to visits.
• Communicate with family or facility staff to coordinate nursing services.
• Create a warm, respectful, calm environment for patients and families.
