Job summary

Patriot Healthcare seeks a compassionate Hospice Certified Nursing Assistant (CNA) to provide personal care per nursing assessments and care plans, under nurse supervision. You will work as part of an interdisciplinary group to implement plans, ensuring patient comfort and dignity in a hospice setting.

Responsibilities include daily care tasks, documentation, and communication with families and staff to maintain continuous nursing services.

• Stewards the philosophy/mission of Patriot Healthcare focusing on dignity and respectful care for patients with life-threatening or chronic illnesses.
• Participates with the IDG in development and implementation of the patient/family care plan.
• Performs tasks per Hospice policies and supervisor instructions.
• Ensures patient comfort and dignity during care.
• Documents tasks performed and communicates with family and staff as needed.
• Maintains warm, supportive environment and clear communication with supervisor.
