Job summary

Patriot Healthcare is seeking a Hospice Certified Nursing Assistant (CNA) to provide personal care for assigned patients in accordance with nursing assessments and the care plan, under the supervision of a nurse. You will work to keep patients comfortable and maintain dignity in a compassionate, supportive environment.

Responsibilities include collaborating with the IDG on care plans, completing tasks per policies, documenting visits, and communicating with families to ensure continuous, quality

Qualifications

• High school diploma or equivalent; CNA certification preferred.
• Hospice or palliative care experience a plus.
• Strong communication and teamwork.
• Ability to document accurately.

Responsibilities

• Provide personal care services per care plan and nurse directions.
• Assist with daily living activities and comfort.
• Observe and report changes to supervisors.
• Document tasks and communicate with families.
• Maintain clean, safe, and respectful environment.
