Job summary

Patriot Healthcare is seeking a Hospice Certified Nursing Assistant (CNA) to provide personal care to assigned patients per the care plan and nurse directions. This role emphasizes compassionate, respectful treatment for patients facing life threatening or chronic illness.

Responsibilities include assisting with daily activities, supporting the care team, documenting tasks, and maintaining a warm, safe environment for patients and families while communicating patient needs to supervising nurses.

Qualifications

• Provide personal care per patient care plan and nursing instructions.
• Communicate effectively with patients, families and staff.
• Maintain patient comfort and safety during care activities.

Responsibilities

• Assist with activities of daily living to ensure patient comfort.
• Support nursing staff in implementing the care plan.
• Document care tasks and communicate changes to the nurse supervisor.
• Maintain a clean, safe and respectful patient environment.
• Observe for changes in patient condition and report promptly.
